The Role
                                               

This role will strive to ensure COVID positive community care pathways are fit for purpose. You will work in partnership with key stakeholders to develop and implement robust primary care pathways that support the care of COVID positive people in the community. 

Key accountabilities in the role are;
  • Ensure effective, appropriate and safe models of care are delivered
  • Ensure community care pathways support at-risk populations
  • Work in partnership with the Local Health District and GP’s
  • Develop consumer understanding and confidence of COVID positive community care
  • Coordinate primary care education and implement community care pathways
  
Skills Required

You have demonstrated experience within the primary/community health setting, aged care, or human services and can translate complex information, policies, and guidelines. You are a strong collaborator, thrive in dynamic environments and are adept at working with at-risk and vulnerable populations, key subject matter experts and stakeholders.
  
Additionally, your background will include:
  • The delivery of projects in complex and specialised environments
  • Relevant tertiary qualification & strong interpersonal, oral, and written skills
  • Strong stakeholder engagement with a proven ability to build relationships
  • Working with a range of IT such as databases for information management
  • NSW driver’s licence and a willingness to travel which may include overnight
  • Ability to work under pressure, prioritise commitments and meet critical deadlines
